- Pictured in the main page: Like in the original source, the Grimm's Fairy Tale Classics version of Snow White has Snow's mother the Good Queen die giving birth to her. It's symbolized by the pictured scene that first shows the dead Queen lying on the bed with her face covered with a cloth, then switches to baby!Snow's cradle: then, the Narrator says something by the lines of "it's as if the Queen had switched places with the child, as she passed away at the same time the baby girl was born". Snow is, from then on, raised by her nanny Doris.
- In the Clannad anime (more exactly, in ~After Story~) and in her VN route, Nagisa dies while giving birth to Ushio. Quite justified since poor Nagisa was already an Ill Girl to start with (and her health took a further nosedive during the pregnancy itself), and she was giving birth at home since a heavy snow fall prevented her husband Tomoya from either taking her to the hospital or bringing a doctor to help her; Sanae and Akio managed to bring a friend of theirs who worked as a midwife and she did what she could, but she was unable to save Nagisa. At the end (and in the VN's True Ending), the Girl from the Illusionary World manages to use the magic of the light orbs... to play around with the timeline and create a new one where Nagisa survives.
- In Lady, it's implied that Sarah's mother/George's first wife Frances (already an Ill Girl) died when having Sarah or very few afterwards.
- Hajime no Ippo has Takeshi Sendoh's grandmother telling the local Hot Scoop Mari that her daughter-in-law/Sendoh's mom died right after having him, strongly implying that she perished like this. No one blamed him for that, however.
- The Literature example from The Secret Garden also takes place in its anime version, Himitsu no Hanazono. Colin's mother Lillias was already a bit of an Ill Girl, then she went up a tree to get a beautiful rose as a birthday gift to her handmaid and protegée Camilla, then she fell off...
- In Another, Kouichi Sakakibara's mother Ritsuko died giving birth to him... as a part of the curse afflicting her younger sister Reiko's school class. She was an Ill Girl before that, which surely didn't help.
- In Tarot Cafe, Pamela's mother was a midwife. A mother and her newborn died during a childbirth she was in charge of, and both the lady and a child Pamela were accused of witchcraft. Pamela's mom confessed so she could get her daughter spared, and was burned at the stake.
- Subverted in Darling in the FRANXX: as Mitsuru is told that his Love Interest Kokoro has given birth and rushes to the birthing room that their friends and companions prepared for her, the whole scene is set up as if this trope has taken place... but both mother and baby (her and Mitsuru's daughter, Ai) are all right. The others were just overwhelmed with emotion since Kokoro is the first of the Parasytes who has ever given birth.
- In Dance in the Vampire Bund, werewolf kids are always born to werewolf dads and human moms, and the latter near always die due to childbirth complications. Akira and Yuuhi's mother is one of the very few exceptions, but after Yuuhi's birth she's in a wheelchair.
- Subverted in GoLion: it's said that Prince Sincline's mother died giving birth to him, but he refuses to believe it because he says that he has seen her in his Bad Dreams, and demands to know what happened to her. It's later revealed that he's right: she was an Altean concubine of Emperor Daibazaal and was murdered by him when Sincline was a very young boy, after failing to get Daibazaal to spare some prisoners..
- In Fushigi Yuugi, it's said that Tamahome's mother died giving birth to his youngest sister, Yuiren. She isn't blamed for it at all, in fact she's the sibling that Tamahome is the closest to.
- Miwako Horiguchi, the mother of the titular character from Ganbare Genki, died giving birth to him. As such, his father Hideki does his best to raise him — until he dies, too.
- In Blue Exorcist, Rin and Yukio's mother Yuri Egin perished like this. The circumstances tend to differ whether we speak about the manga or the first anime, however.
- Quatre Raberba Winner's mother Catherina died this way in Gundam Wing, albeit on her own terms. She and her husband Zayeed had many daughters born from artificial wombs, and Catherina knew she was too frail to carry a child and live, but she knew the risks and wanted to carry her husband's child just once. Quatre has been kept Locked Out of the Loop about it, likely so he will not blame himself for his mom's death.
- The titular character's mother in Hana no Ko Lunlun passed away like this, as seen in a flashback. She died begging her husband and soon-to-be widow to not blame the baby girl for her demise, and he did not. For as long as he lived.
- Bokurano has Jun Ushiro bullying his younger sister Kana because their mother died giving birth to her. It turns out to be more complicated, though: Jun is adopted, and his birth mother is still alive and had to give him away for quite serious reasons.
- Kodomo no Omocha has Akito's mother Koharu losing her life due to his birth. It's the reason why his father Fuyuki buries himself in work and his traumatized sister Natsumi bullies him, until Sana finds out and helps them reconcile.
- Bleach: The anime-only character Rurichiyo Kasumiouji lost her mother like this.
- The Can't Fear Your Own World novels state that Aura Michibane's mother died during her child's birth due to old injuries that she, a human woman, received when still pregnant with her.
- The Literature case from Treasures of the Snow also counts for its Animated Adaptation, Story of the Alps: My Annette. After Annette Burnier's mom dies giving birth to her little brother Daniel, she becomes very protective of him...
- Elfen Lied has Mariko's mother Hiromi. She already was an Ill Girl who barely survived giving birth to her girl due to cervical cancer, but when she found out that her husband wanted to kill Mariko for being a Diclonius, she chose to make him change his mind rather than going through the urgent surgery that could've saved her. So when Hiromi was begging Kurama to spare baby Mariko, the surgical stitches she had from the complicated birth went undone, she collapsed and ultimately bled to death.
- In Digimon Savers, the birth of Touma Norstein's beloved half-sister Relena was extremely hard on both mother and baby, and the Norstein clan's leaderess (Touma and Relena's grandmother) ordered the doctors to do what they could to save the baby's life since Touma was an Heroic Bastard. Relena survived, her mother did not; it's also heavily implied this is the cause of poor Relena's Ill Girl status.
- According to the Demon Slayer: Kimetsu no Yaiba fanbooks, Gyomei "Stone Hashira" Himejima's mother died giving birth to him.
- In Voltes V, Prince Heinel's mother Lady Lozania died giving birth to him... while his father Prince Lacour was unjustly imprisoned.
- Horribly and weirdly used in Berserk: Sys and Gambino found a newborn Guts at the feet of a gallow where several corpses were hung, including that of his birth mother. This means, either Guts' mom was executed for whatever reason right when she was due and then her lifeless body somehow gave birth to him anyway, or (most likely) she died of childbirth/was executed right afterwards and then her newborn son was thrown at her corpse's feet and left to die...
- In another just as horrible twist, Emperor Ganishka created his army by tossing pregnant women into a pool of liquid from captured Apostles. The babies become Fetus Terrible and then burst from their mother's stomachs Alien style, killing them.
- In Yu Yu Hakusho, the unnamed "shaman cannibal" that Yuusuke Urameshi's demonic ancestor Raizen slept with died giving birth to her and Raizen's kid. Raizen himself was so heartbroken over her death that he decided not to ever eat one single human again until he knew she had been reincarnated. He ended up basically starving himself to death to keep his promise, dying right after he explained the whole deal to Yuusuke himself.
- Subverted in Fairy Tail. Makharov's mother Rita died almost immediately after he was born, and it's believed she fell victim to this. . . but in reality, what killed Rita was an extremely nasty curse that befell her husband Yuri Dreyar's best friend and Fairy Tail co-founder, Mavis Vermillion, since said curse would make Mavis immortal while killing those close to her.
- In Oshi no Ko, the birth mother of Dr. Goro Amamiya (Aquamarine "Aqua" Hoshino's past self) died like this. In her desperate attempts to hide her pregnancy from her parents she decided to give birth alone at home, suffered complications and died of blood loss. He was Raised by Grandparents from then on, and it's implied that his grandfather blamed him for his mom's death.
- In Jinrouki Winnvourga, many of the women captured as Sex Slaves by the Dominator Empire end up in People Farms and made into breeding slaves, especially if they rebel against their "masters". One of these breeders is the protagonist Mashiro's close friend Mito, who at some point witnesses how a young woman collapses right next to her after the poor woman's Child by Rape is born and then is immediately taken away from her; she's horrified and screams at the scientists around to get help for the dying mother, without any success...
- Subverted in Black Clover: Noelle Silva's mother Acier was severely injured in an attack that took place the day of Noelle's birth and died as a consequence of it, which her other kids blame thew poor girl for -- but in reality, she was killed by an horribly powerful curse by the demon Megicula. The only one who knew was Noelle's Aloof Big Brother Nozel, who then spent 16-17 years planning how to avenge their mom and release her soul from Megicula's clutches... and got to do it with Noelle's help.
- Downplayed in The Apothecary Diaries. The Virtuous Consort Lishu's mother survived her very risky childbirth, but suffered injuries that rendered her an Ill Girl and caused her death when Lishu was four. Lishu blames herself for it and keeps her mom's mirror as a Tragic Keepsake.
- In Captain Tsubasa, Tsubasa's JPN National Team companion Gakuto Igawa lost his girlfriend Caterina like this. He does his best to raise his daughter Lisa (albeit at times he needs his older brother Hayato and his wife Noriko's help) and the little girl clearly adores him.
- As of CT Rising Sun: Finals, Tsubasa's Victorious Childhood Friend Sanae gave birth to their twins... after having taken a Staircase Tumble and being severely injured. She han't fallen victim to this yet, but is in a coma and there IS a chance for it; the local sort-of Waif Prophet, the Spanish player Michael, sees her soul almost ascending to Heaven and he goes all "Please Don't Leave Me!" at her...
